A Pilot Study of Extended Adjuvant Therapy with Metronomic Docetaxel for Patients with Operable Triple-Negative Breast Cancer
Studio pilota di docetaxel metronomico in adiuvante nei casi tripli negativi.Lo studio dimostra la fattibilità di un prolungamente della chemioterapia con basse dosi settimanali docetaxel nella pazienti che hanno ricevuto un trattamento standard adiuvante per un ca mammario triplo negativo operato. Limiti dello studio:
1) casistica non adeguata;
2) il docetaxel è un farmaco poco adatto ad una terapia metronomica in ragione della tossicita midollare del farmaco e delle onicopatie gravi da trattamento prolungato; più meta-analisi hanno dimostrato che si può potenziare il trattamento adiuvante dei casi triplo-negativi ,ottenenendo un umento della sopravvivenza, inserendo la capecitabina (anche metronomica) dopo il trattamento adiuvante classico con antracicline e taxani.
3) nei casi triplo negativi è altamente raccomandabile un trattamento neo.adiuvante a cui far seguire una terapia con capecitabina nel caso di malattia residua (studio Create-X) con ottimi risultati di sopravvivenza a 5 anni. -
Abemaciclib plus trastuzumab with or without fulvestrant
Studio randomizzato di fase II che confronta nelle pazienti HER2+ ER+: abemacilib+trastuzumab + Faslodex vs abemaciclib + trastuzumab vs chemioterapia+trastuzumab in donne che avevano ricevuto almeno due linee di trattamento anti-HER2 per malattia metastatica.
Lo studio dimostra un vantaggio in PFS (8,3 mesi vs 5,7 mesi) tra il primo gruppo ed il terzo; il secondo gruppo ha una PFS di 5,7 mesi.
Le pazienti HER2+ER+ sono le meno responsive alla terapia anti HER2 ed un trattamento anti-ormonale si è dimostrato utile in altri lavori anche se non in terza o quarta linea. In questo studio il solo abemaciclib non è sufficiente senza una terapia ormonale e questo è abbastanza scontato. Il potenziamento della terapia ormonale con abemaciclib svolge un ruolo clinico, associato a trastuzumab, e questo è in asse con il dato di efficacia delle combinazioni faslodex+inibitori delle cicline. Lo studio è una "proof of concepts" e ha scarsa utilità clinica; forse questo approccio potrebbe tornare utile nella terapia ormonale adiuvante dei casi HER2+ER+ e nel mantenimento ormonale dopo terapia di prima linea della malattia metastatica HER2+ER+ -

Network metanalisi delle seconde linee ormonali nella malattia metastatica ER+.
La metanalisi segnala come trattamenti più efficaci faslodex+inibitori delle cicline e everolimus + exemestane.
I risultati confermano ciò che è emerso dagli studi randomizzati.Di fatto faslodex + inibitori delle cicline è la seconda linea dellle pazienti che hanno ricevuto un inibitore delle aromatasi e everolimus+examestane è la seconda linea possibile dopo ormonoterapia+inibitori delle cicline. - RADIOTERAPIA (Rovea)

In questo articolo emerge che le bevande zuccherate (compresi i succhi di frutta puri) correlano con un aumentato rischio di cancro. Il dato nuovo rispetto alle conoscenze precedenti è che questa correlazione è indipendente dalla obesità, e da altri fattori noti.
Si tratta di una indagine prospettica condotta su 101257 participanti. che indaga sulle abitudini alimentari e il rischio di sviluppare patologie.
Già era nota la correlazione con il consumo di zucchero e le malattie metaboliche e cardiovascolari.

Lactation during and after Breast Cancer
In questo articolo sono presi in considerazione i rischi legati alla lattazione dopo trattamento per cancro mammario poiché è noto che l’allattamento al seno è un aspetto importante della relazione madre-neonato ed è di grande beneficio per il bambino. Sfortunatamente, molti farmaci presi dalla madre possono passare nel suo latte ed esercitare un effetto sul neonato. Sono disponibili dati molto limitati ed è giustificato evitare allattamento in corso di chemioterapia, trattamento ormonale e agenti bersaglio recentemente introdotti, nonché anticorpi monoclonali. Nei casi in cui il trattamento è terminato, l'allattamento al seno è consigliabile ogni volta che è possibile, anche se i pazienti che hanno ricevuto radiazioni mammari potrebbero non essere in grado di produrre una quantità sufficiente di latte su quel lato.
